Inquiries and consultation regarding disclosure requests
- If you wish to consult with us about your personal information, please first come directly to the Information Disclosure Office.
- When consulting with the FOIA Office regarding personal information, please be sure to bring proof of your identity (driver’s license, insurance card, alien registration card, personal number card, or other publicly issued document).

Request for disclosure, correction and suspension of use, payment of disclosure request fee, and decision on disclosure or nondisclosure, etc.
- There are three types of personal information that can be requested: disclosure, correction of disclosed personal information, and cessation of use of personal information.
- After confirming that the request for disclosure identifies the personal information held by the University, the applicant is required to submit the prescribed request procedure to the Information Disclosure Office.
- The fee for disclosure is 300 yen per corporate document. (The fee for affiliated hospitals is determined separately.) No fee is required for correction of disclosed personal information or suspension of use of personal information.
- Upon receipt of a Request for Disclosure of Retained Personal Information, etc., the Information Disclosure Office will send the contents of the request to the relevant university departments, etc., and make a decision on disclosure or nondisclosure after reviewing the request in light of the criteria.
- A response to the decision will be made within 30 days from the date of the request. However, if a decision on disclosure cannot be made within 30 days due to administrative difficulties or other justifiable reasons, the period may be extended for up to another 30 days.
- In either case, the individual making the request will be notified in writing.

Anonymously Processed Information of Administrative Organs, etc.” in which personal information held by administrative organs, independent administrative agencies, etc. is processed so that specific individuals cannot be identified and such personal information cannot be restored, is to be provided to business operators, etc.
